SinusMonitor app for iPhone and iPad

For patients of Dr. Janalee Stokken, Dr. Erin OBrien and Dr. Garret Choby to submit information about their sinus disease symptoms periodically. This is not a substitute for routine clinical care but responses may be used to track their symptoms of rhinosinusitis and plan future care. Responses will be used for research purposes (MCR IRB# 16-001409.)

If your symptoms are worsening, in addition to using this survey, please also contact your provider to schedule a follow-up appointment as soon as possible. If you have an urgent medical problem, please call your rhinologist, your primary care provider or an urgent care facility immediately.
